How do I measure?

The length of the old rail on my undermounts is 20 in, is that it? I want it to open a bit more and was going to go 21. Does that sound correct for sizing?

This particular slide comes in a 21" Slide . The outside member is 21 1/2" and the actual member that attaches to the drawer is 20 1/2". In saying that you may already have a 21" slide. The next size up would be 24" but you would then have to rebuild your drawer box. This type of slide requires that the drawers be built based on the specs of the drawer slide.
